A practical guide to the information you choose to share. Use the planner to prepare your conversation, or skip it and speak directly with the team.

- Selecting an area does not send an appointment request.
- Refreshing or closing the page clears unsubmitted planner and form details.
- You can ask about data already received by the clinic by email.

## 1. Before you send anything

The planner holds selected areas, concerns and your optional note in page memory. The form also holds the contact and scheduling details you type. The site does not save these in local storage, session storage or cookies. Moving between views keeps choices while the same page remains open; navigating elsewhere, refreshing or closing it can clear them.

Browsing still makes technical requests to the hosting service. Some treatment links carry an interest identifier in their URL, which can appear in browser history and server logs. Keeping the planner in memory does not mean browsing is anonymous.

## 2. What an enabled form sends

Sending an enabled request transmits your name, email, optional phone number, preferred date and time, preferred language and privacy acknowledgement to the website's request service and the clinic mailbox. Selected areas, concerns, treatment interest and your message are included only if you tick the separate optional sharing consent. Without it, those fields are omitted. Review the choices before submitting; you can request an assessment with contact details only.

If online submission is unavailable, the form states this and does not send a request. A success receipt confirms receipt of the request, not an appointment. A separate automatic email acknowledgement contains only the reference and general next steps; it does not repeat private form details. The clinic must confirm availability separately. A network error can leave receipt uncertain; use any reference provided when asking the team to check.

## 3. Edit or clear your choices

Deselect an area or concern to remove it from the current request. Use the interest selector to change a prefilled treatment or device, and edit or erase the message before sending. Refresh or close the page to discard unsubmitted details. Clearing the current page does not withdraw an already received enquiry.

If a URL contains a treatment interest, opening it again may prefill that interest. Use the main assessment link without its query parameters to start without that prefilled choice. Browser history, saved links and browser autofill are controlled separately in your browser settings.

## 4. Keep health information for the consultation

The explorer is an optional conversation aid, not a diagnosis, medical record or treatment recommendation. For an initial enquiry, contact details and preferred dates are enough. Discuss medicines, allergies, previous procedures and medical history directly with the professional through the appropriate channel.

Clinical photographs and consent to treatment are separate from website choices. Ask how clinical records are stored, who can access them and how long they must be kept. Permission to use a photograph for publicity is separate from providing care.

## 5. Ask about information already received

Email liberdade108@valdoleirosclinic.com with the right you wish to exercise and enough information to locate the enquiry, such as its date or reference. Do not attach an identity document or medical file unless the team explains why a proportionate check is necessary. Keep a copy of your request.

You may request access, correction, erasure, restriction or portability, object where applicable, or withdraw consent. Legal limits may apply, particularly to clinical and billing records. A response is normally due within one month; a justified extension must be explained within that first month. The privacy policy explains the complete scope and CNPD complaint route.
